After Care and Support

Upon completion of the program, the multi-disciplinary team who cared for you and your baby meet to review your progress and goals achieved. If necessary, recommendations and referrals for further support will be provided. At the conclusion of the program an evaluation is also conducted.

Within the fortnight after discharge, staff from the Illawarra Early Parenting Centre provide a follow up call to discuss goal progress and problem solve any issues that may be encountered.

The Illawarra Early Parenting Centre will liaise with your family GP and it is recommended you continue to see your GP or local Child and Family Health Nurse for further advice and guidance. Should you experience ongoing difficulties, the Illawarra Early Parenting Centre can offer streamlined re-admission to day program support and education.
